Health Insurance

Depending on your plan details, your health insurance plan may not cover part or all of your treatment costs.

​

In some cases, we are unable to accept Medicaid patients or certain insurance carriers. Please ask when you call to be sure your care will be covered.

Please contact your health insurance plan for questions about what’s covered and how much your co-pay amount will be.

 

Please note that we collect at the time of service.

There is an additional fee for completion of forms for use outside of the patient's medical record, including:

Self pay

If you are planning to pay for your visit yourself, the office staff will collect the full amount due at the time of your appointment, unless an arrangement has been made prior to visit.

For amounts not collected at time of service, you may pay your bill by mail or phone. You can also conveniently pay your bill online using a credit card.
